If you need to rehome your dog/cat, please complete this form in its entirety and follow the instructions for submitting photos. Your dog/cat needs to be in good health, have UTD vaccines, be spayed/neutered, and do well with people. All animals will be evaluated for behavioral concerns prior to owner surrender.

What is your reason for surrender?

Has your dog/cat had any vaccinations or heartworm testing? Has it been altered (spayed or neutered)? If so, please describe.

I, the owner, understand that a current health check (1-2 months), all vaccinations (DAPPV, Bordatella, and Rabies), and spay/neuter of my pet are my responsibility before my pet can be rehomed.

I, the owner, will continue to house and care for my pet for the duration of the rehoming process. Whether it takes weeks or months to find him/her, I will continue to house and care for my pet. I understand that at no time will Kenai's House take in my pet unless a suitable foster is found.

I, the owner, will aid in the rehoming process by providing photos and videos and participating in adoption events (transportation to and from). If I am unable to get my pet to an event, I will let Kenai's House know 3 days in advance to make the appropriate arrangements.

Rehoming Agreement Disclaimer: By submitting this application, you agree that Kenai’s House is providing rehoming services with the understanding that we are the sole rescue organization handling your pet’s placement. If you choose to break this agreement by rehoming your pet through another rescue, shelter, or individual, you agree to pay a $500 service fee for the time, resources, and support provided by Kenai’s House. Our time is valuable, and this policy ensures we can continue helping animals in need responsibly and sustainably.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Kenai’s House, Inc., and its members from any claims, damages, costs, or actions incurred because of the care or actions of my dog.

I waive all rights and claims against Kenai’s house regardless of the expense or damages incurred during the entire duration of the rehoming process. I understand I take on legal ownership of the dog until he/she is rehomed. I will not pursue legal action against Kenai’s house and will not hold them responsible in the event that any injuries to persons or damages to property arise.
